Band Forms

In order to distribute and gather vital information to/from our students and families, the Frenship Band has several required forms that must be completed to stay in compliance with local, district, and state policies.

The following forms will be completed online by accessing the Rank One website HERE.

  • Preparticipation Physical Evaluation (PPE) - Medical History

  • Insurance, Emergency, and Medication Information

  • Parent/Student UIL Marching Band Acknowledgement Form

  • Extracurricular Code of Conduct

  • Band Handbook

The Rank One website will require you to create a separate login, verify your email, and navigate to the forms above.

The following form will be downloaded from this website HERE.

Print this form, take it with you to your doctor, and have him/her complete the form.  Note, the physical examination must be completed ON THIS FORM.

 

Once completed, bring this form to the band hall and turn it in to the Band Staff in person.  This form will be kept on file in the band office.
